Investigation Summary
On September 28, 1999, Employee #1, working for a manufacturer of plastic bags, was using a rag to clean the drum of a printing press. He dropped the rag, and as he picked it up, his right hand became caught between the large roller and the 2 in. drive shaft. The motion of the machinery forced the rag back into his hand and prevented him from extricating himself. He sustained serious hand injuries, for which he was hospitalized.
