Employees fingertip amputated between belt rollers

On March 2, 1999, Employee #1 was using a putty knife to scrape plastic residue from the tops of the 1-1/4 in. wide belts of a plastic bag-making machine. His left little finger became caught between the belt rollers, amputating the distal joint. Employee #1 was cleaning the machine while it was running.
