Investigation Summary

Investigation Nr: 170604623
Event: 04/10/1998
Employee injured when struck by falling pallet load

Employee #1 was part of a crew off loading ten pallets of Unistrut material into a concrete alcove. They were using a 980 loader with fork attachments to maneuver the ten 2,800 lb, double-stacked pallets. The top pallet slipped off and pinned Employee #1 against a broken concrete wall. He suffered a fractured femur and pelvis, for which he was hospitalized. Apparently, the bottom pallet was broken and timber blocking had been put under it. As a result, the load of pallets settled unevenly and the top pallet slid off.
