Employee's foot injured when caught in screw conveyor

On October 26, 1999, Employee #1, age 18, was removing straw from the straw collector at the top of a rice cleaning machine. He was stepping down from a drop chute flange onto the screw conveyor frame when his right foot slipped on some rice grains and became caught in the moving conveyor. Employee #1 sustained deep lacerations on both sides of his foot, for which he underwent surgery. The screw conveyor was not guarded. Employee #1 was a temporary agency worker at a company that dries, mills, stores, and packages rice,
