Investigation Summary
On August 13, 1998, Employee #1 was walking under a scaffold in an auditorium when he fell 8 ft 4 in. through a floor opening into the concrete orchestra pit. He sustained serious injuries, for which he was hospitalized. The lighting in the auditorium was poor and the floor opening was not guarded. Employee #1, a civil engineer and president of the corporation, was at the job site to inspect some welds.
