Employee's foot injured in mill cant

Employee #1 had a short cant that became hung up and was causing the mill cant chain to jam. The chain is governed by a HOLD-DOWN button, and Employee #1 used a stick to wedge the button into the 'on' position. He then got on the running chain to pry out the short cant. When the short cant came free, Employee #1 lost his balance and another cant trapped his foot against the roll case, which was receiving the cants from the cant chain. His foot sustained injuries, for which he was hospitalized. Employee #1 was an experienced mill cants button operator. This accident resulted from his overriding the HOLD-DOWN button and from his mounting the machine as it was running, instead of locking it out.
