Investigation Summary
Employee #1, a superintendent for a plumbing and heating contractor, was engaged in sewer line work in a 20 ft long by 8 ft deep by 2 ft wide trench when one side of the trench slowly broke away and collapsed. Employee #1 was buried up to his chest and struck on the head by a piece of asphalt. The local fire department spent several hours rescuing Employee #1, who had four toes amputated and also sustained head injuries and multiple contusions. Employee #1 has no memory of the accident, but according to a witness it occurred during his second time in the trench. The trench was neither shored nor sloped.
