Employee killed when crushed by scissor lift

Employee #1, a millwright at a sawmill in Fortuna, CA, was repairing the drive chain on a conveyor system. He locked out the conveyor system on which he was working and positioned himself next to and under the edge of a scissor lift that moved timbers away from the head rig. Employee #1 was placing the chain on the sprocket when the scissor lift was activated by a coworker who was moving a 16 ft long 12 by 12 timber off the head rig roll case. The scissor lift came down on his back, crushing him. Employee #1 was killed.
