Investigation Summary
Employee #1 working as a press operator for a manufacturer of egg cartons and other paper products was attempting to clear a jam. She did not stop the press, but relied on a push button switch to stop the machine. The push button switch did not stop the press and as she reached into the press, three of her fingers on her right hand were amputated and she was hospitalized. Employee #1 failed to lockout the press prior to clearing the jam.
