Employee's leg broken when caught in machine rolls

Employee #1, the grader at the planer chain at a sawmill, had just finished a run of 2 by 4 lumber. He was on the planer chain clearing it off when he realized the sweed rolls should be adjusted for 2 by 6 lumber. This was normally done by having an employee stand on the chain to facilitate loosening the lock nut and turning a hand wheel. The procedure was carried out with the sweed stopped, but not locked out. Employee #1 decided to loosen the lock nut and to try adjusting the hand wheel with the sweed running because they sometimes had to run it to make the wheel turn more easily. He put his right leg over a 20 in. gap and onto the sweed, trying unsuccessfully to adjust it. Employee #1 then decided to shut off the sweed. He was turning it off when he slipped and the rolls grabbed his leg, pulling him into it. Employee #1 suffered a broken right leg that required surgery. The cause of the accident was not stopping and locking out the sweed prior to making adjustments.
