Employee's elbow injured in fall at egg carton plant

Employee #1 was the crew leader in the egg carton department at a plant that manufactured egg cartons and other paper products. On February 5, 1998, he was in the basement dye room, refilling the vats that supply dye to the egg cartons being made on the upper level. He was preparing to climb the small flight of stairs to the vat when he slipped on the floor and fell, sustaining a small cut and bruise to his left elbow. He received first aid and was sent back to work. Approximately 12 days later, Employee #1 re-injured his elbow when he bumped it against an egg carton machine wrapper. Within a few hours, the elbow became inflamed and swollen; Employee #1 was hospitalized for four days of treatment.
