Investigation Summary

Investigation Nr: 170813984
Event: 03/19/1996
Employee's toe injured when caught in conveyor rollers

At approximately 6:00 p.m. on March 19, 1996, Employee #1, of the Campbell Soup Company, was working on the chicken noodle soup production line. Boxes of frozen cubed chicken were being broken in the section of the conveyor rolls that raised vertically to insert the breaker bars into the boxes. The section had elevated 4 to 6 in. and then stopped because the boxes would not fit into the cage area around the breaking section. Employee #1 stepped up onto the nonpowered roller and straightened the boxes so they would fit. While he was on the rollers, a coworker activated the DOWN control button, lowering the raised section. Employee #1's right foot became caught between the elevated part and the conveyor rolls, crushing his big toe. First aid was provided and he was transported to Sutter General Hospital. An infection developed, and the toe was amputated on March 24, 1996.
