Employee's fingertips amputated in packaging machine

On November 15, 1996, Employee #1, an experienced production worker, was changing the wrapping film on a Pioneer packaging machine for boneless turkey breasts. The normal procedure was to stop and lock out the unit, and to then attach the new film to a remnant of the old film roll. Employee #1 attempted to feed the new film by hand while a coworker jogged the packaging machine. Employee #1's right index and middle fingers became caught between a vertical sheet metal panel and the rising conveyor pocket. The tips of both fingers were amputated, including some bone loss. The employer was cited for a general violation of T8CCR 3314.
