Employee's fingers amputated in vacuum packing machine

Employee #1 was working on the chicken parts packing line next to a Dixie Vac 2000 packaging machine. She informed her supervisor that the machine was not working properly because the plastic film repeatedly became jammed. She was reaching into the machine to pull out the film when the machine cycled, pulling in her left hand and amputating her middle, ring, and little fingers. Employee #1 was transported to the hospital, where two of her fingers were re-attached. At the time of the accident, the machine was being serviced and was not locked out.
