Investigation Summary
Employee #1 was using a wood splitter to split fire wood for a customer. This required his holding a log with one hand while using the other hand to operate the machine. As he was working, a log slipped. Employee #1 was trying to straighten it when the machine caught his fingers and smashed them. His fingers were surgically amputated several days later.
