Investigation Summary
Employee #1 was operating a Crown Reach truck at a distribution warehouse for electronic products. He was locating merchandise to fill an order when he lost control of the vehicle while backing up. The truck crashed and Employee #1 sustained injuries to his back and pelvis. Causal factors were the employer's failure to enforce the forklift safety orders, and failure to maintain the fork truck in a safe operating condition. Employee #1 was working alone at the time of the accident.
