Investigation Summary

At 9:30 a.m. on February 9, 1998, Employee #1, a service representative at a work location in Berkeley, CA, was entering a restroom when she lost her footing and fell onto the tile floor. She suffered a broken left femur and shoulder and was taken by paramedics to Alta Bates Hospital, where she was admitted for treatment. Employee #1 was alone at the time of the accident. She has a prosthetic left knee, which was the probable cause of the accident; she later stated that she felt her left knee go out from under her and could not stop her fall. There was no water or other substance on the floor that might have made it slippery, and the tile, which was of the normal type used in restrooms, was not polished. No accident-related citations were issued.
