Employee killed in fall through skylight

Employee #1 was part of a labor crew from a temporary agency (primary employer) contracted for by a roofer (secondary employer) to resurface the existing flat plane roof on a 200,000 sq ft manufacturing building. Employee #1 was operating a gas-powered blower to clear away dirt and gravel when he lost his balance and fell 30 ft through a plastic skylight cover onto a concrete floor. He suffered numerous traumatic injuries and was taken to Elden Hospital, where he was pronounced dead. Causal factors in this accident include: the secondary employer's failure to guard or cover the skylights with approved material; the primary employer's failure to evaluate the job site for potential hazards and to require any hazards be corrected before allowing its crew to start work; and the primary employer's failure to provide training on site-specific hazards to its employees.
