Event: 06/13/1997
Employee killed when run over by forklift

At approximately 1:00 p.m. on June 13, 1997, Employee #1 was in the bare chassis storage area between sections D-330 and GG-300. He was showing the operator of forklift #6117 where to move a reefer. The coworker looked to see where Employee #1 was standing, then backed up and swung the forklift/container handler to the right. He heard another coworker, the operator of forklift #6118, which was parked about 100 ft away, blowing his horn and yelling over the radio for him to stop. The first coworker stopped, climbed down from his cab, and walked around the swing end of the vehicle. He found Employee #1 crushed under the left rear tire of his forklift; he died as a result of his injuries. It was not determined why or when Employee #1 had moved into the swing radius of the forklift wheel. The second coworker had only seen Employee #1 as he was struck by the forklift. No accident-related violations were found.
