Investigation Summary
At approximately 11:30 p.m. on May 30, 2002, Employee #1 was in the truck yard of a warehouse facility immediately outside of the dock. Employee #1 was dumping garbage in a large yard box. Employee #1 was operating a Toyota lift truck to transport the portable dumpster. The dumping lever on the dumpster was broken, and equipment was not properly maintained. Employee #1 sustained the partial amputations of his left index ring and pinky fingers and was hospitalized.
