Investigation Summary
At approximately 2:00 p.m. on July 6, 1999, Employee #1, a 39-year-old carpenter framer, fractured some bones after falling approximately 21 feet through an engineered roofing system to the concrete floor. The cause of the incident was material failure due to the lack of glue in the Purlin web. Employee #1 fell through the roof frame when the supporting Purlin beam failed. She went to a local hospital for treatments.
