Employee injured when struck by falling stack of cartons

Employee #1 was working on the production line for Dri-Loc, a plastic packaging. He was using a hand-operated pallet jack to move a single pallet of unused Dri-Loc cartons to the area on the line where the finished product was boxed. He became caught on a three-pallet stack and the top stack, which weighed approximately 400 lb, fell over on him. Employee #1 sustained a lung contusion, fractured ribs, and neck and ligament damage. The accident was the result of material stacked in an unsafe manner, a violation of T8CCR 3241.
