Employee hospitalized in fall from ladder

On August 9, 2001, an employee was climbing down from a flat roof after servicing a roof-mounted air conditioning unit. After the employee placed his second foot on an aluminum extension ladder, the ladder's feet slipped outward. The employee fell 10 feet to the ground and was hospitalized with a broken pelvis. The ladder was not secured properly to prevent slippage on the gravel surface.
