Investigation Summary
At approximately 2:00 p.m. on January 8, 2007, an employee was cutting a piece of wood 1.5-in. wide by 7-in. long by 1-in. thick with a shaper, Hammer Model 500 RPDC, when the shaper's cutting head, top table, kicked back on the wood and caught the employee's left thumb and right index and middle fingers, resulting in amputations. The employee was hospitalized and treated for her injuries.
