Employee suffered head injury from being caught between pipe

On March 13, 2002, Employee #1 was inspecting a 36-in.-diameter pipe for defects on the ground level at the north end of the inspection platform, approximately 20 ft east of designated inspection area not visible to any one. Employee #1 entered a prohibited area unaware of incoming pipes and his head was caught between two pipes. Employee #1 suffered a fractured scull and facial lacerations and was hospitalized.
