Investigation Summary

Investigation Nr: 170841472
Event: 06/18/2001
Employee Fractures Ankle And Hit On Head By Falling Planks

Employee #1, a contract employee from CLP Resources, was standing on the plank on its side section, pushing and pulling the center section planks, one-at-a-time. He was working approximately eleven feet high. The metal pump-jack units were on doubled two by four uprights with metal bracings. The planks tipped when Employee #1's feet slipped out onto the ends of the unsupported planks, beyond the pump-jacks. Employee fell to the ground, fracturing his left ankle and was hit on his head by falling planks. His injury was minor, because he was wearing a hard hat. His employer failed to ensure that the planks were secured to pump-jack ledges. Asbestos and lead abatement were done on multiple residential structures. Employee #1 went to a local hospital for treatment.
