Employee's Finger Is Amputated Fixing Chair Lift

At approximately 12:25 p.m. on October 20, 2006, Employee #1, an auto mechanic, was repairing a wheelchair lift, a LIFT-U Model L012, on a 1990 Orion bus, Coach Number 9029. The curbside barrier of the lift was not staying in the up-right position, 90 degree as measured from a horizontal plane. It was flipping slightly backward, a defect condition known as fanning. The curbside barrier was in the up-right barrier position, and the curbside cylinder rod was in an extended position. Employee #1 was adjusting the angle of the barrier by loosening the curbside lift cylinder jam nut located inside the bracket linkage using his left thumb and index fingers. He failed to use a block or special bracket on the moveable part to prevent it from inadvertent movement, and failed to use an extension tools such as a wrench. The jam nut fell causing the barrier to drop which pulled the bracket linkage closed, pinching the tip of his left thumb. Employee #1 was hospitalized and underwent amputation surgery on his left thumb.
