Investigation Summary
Employee #1, the foreman, was replacing the joist on an angle iron in preparation for roof re-sheathing on a tilt-up concrete building. He had his foot on the joist and was using a hammer to tap it in when the joist hanger slipped. Employee #1 fell approximately 12 ft, fracturing his leg. The cause of the accident was the foreman not recognizing the possibility that the hanger could slip because it was not nailed to the joist.
