Investigation Summary
On March 22, 2002, Employee #1 was pureeing tomatoes in a Reeves food disintegrator at a State facility that cares for and trains developmentally disabled individuals. When the machine jammed, she reached deep into intake chute to clear it, injuring her finger. Employee # 1 was hospitalized for an amputated right index fingertip.
