Employee injured when struck by crane load

Employee #1 was power washing the base of a truck-mounted, 14,000-pound capacity National crane, Model 5T27, Serial Number 6627, by standing on the bed (frame) of the truck. The flat metal bed was hinged at the rear and suspended under load from crane's hook. The crane controls were unattended and the crane failed, dropping the load onto Employee #1, causing fractures to his head.
