Employee's Finger Crushed by Falling Engine Part

At approximately 8:30 a.m. on July 19, 2002, Employee #1 was in a service pit replacing a leaf spring assembly on a front loader refuse truck. It was dark in the service pit below the truck and Employee #1 was using his right index finger to check the alignment of the bottom of the centering bolt of the leaf spring assembly, which had a hole on top of the axle beam. The leaf spring assembly was designed to settle into place when the bottom of the centering bolt and the hole on top of the axle beam were aligned. When this happened, Employee #1's right index finger became trapped and was crushed. A coworker raised the hoist, allowing Employee #1 to withdraw his finger, part of which was subsequently amputated.
