Investigation Summary
Employee #1 was cleaning out the chute of a Werner & Pfeiderer-Hatton dough chunker machine in a bakery. She became pinned when the machine cycled while she was inside. Employee #1 suffered severe bruising of her left hip and upper leg, for which she was hospitalized. The load was on the wrong circuit and no blackout occurred
