Employee #1 was starting up a Hinds-Bock food processing machine to batch and pack meals She was cleaning and clearing the unit when the machine valve rotated, amputating the tip of her left index finger. She had not locked out the machine prior to cleaning it. The employer was cited for a serious accident-related violation.
