Employee's thumb tip amputated by forklift

Employees #1, a truck driver, and a coworker were adjusting a three-unit load of plywood on the bed of a 1994 GMC 3-axle lumber truck to eliminate any side overhang. The coworker was operating a forklift with a T-track extension, and Employee #1 was holding a 2 by 6 sleeper between the forks and the plywood unit. The coworker pushed on the sleeper before Employee #1 could get his hand out, and the tip of his right thumb was amputated. The accident resulted from miscommunication between Employee #1 and the coworker as to when to push the forks against the sleeper.
