Investigation Summary
Employee #1 was carrying air conditioner filters along the wash-out pad when his head struck a hand-operated winch mounted overhead. He suffered head lacerations and lost his balance, getting his foot caught between some pipes that were lying in the walkway. He fell and fractured his leg. The winch was mounted low, below Employee #1's height of 68 3/4 in.
