Investigation Summary
Employee #1 and two coworkers were on the sloped roof of a multiple-building apartment complex, replacing siding on the chimney as part of a reconstruction project to replace damaged or deteriorated components. Employee #1 was standing at edge of the approximately 5-in-12 pitched roof inspecting the siding for warpage when he fell approximately 20 ft 4 in. to the ground. He sustained a broken back, a broken nose, and facial fractures. Employee #1 and his coworkers were not provided with any form of fall protection, such as safety harnesses and lanyards.
