Investigation Summary
Employee #1 was clearing a log jam in a debarker unit at a sawmill. He was standing on top of and straddling the log slip and a transfer chain with 4 in. square links when his right foot slipped, crushing and fracturing his three middle toes. Employee #1 was not clearing the jam in the manner specified by his employer. At the time of the accident, the debarker was reasonably free of debris.
