Employee's foot crushed by falling battery, later amputated

At approximately 2:15 a.m. on November 12, 1996, Employee #1 was in the company battery maintenance room, changing the battery for his forklift. He had removed the dead battery and was replacing it with a fully charged one when the battery fell, crushing his left foot. Initially, he sustained a severe injury to his large toe and the forward part of his foot. Later, however, it was determined that the entire foot required complete surgical removal. The battery fell when it either slipped from the safety hook, or the safety hook broke from the motion of the electric hoist.
