Investigation Summary

Investigation Nr: 201790367
Event: 11/12/1997
EMPLOYEE INJURED BY FALLING CONTAINER

On November 12, 1997, Employee #1 was working with another employee exchanging old trash containers for new trash containers. Employee #2 was using a front-end loader to pick up old trash containers from the dock area, stage the old containers in the parking lot area, and then pick up new containers from the back of the delivery truck. During this operation Employee #1 was standing next to the truck listing the serial numbers of the trash containers in the staging area. As Employee #2 was backing away from the delivery truck with the last container, Employee #1 was stooping down and was not visible to the vehicle operator. The front-end loader contacted a trash container and the container tipped and fell onto Employee #1. The container was lifted off Employee #1 and he was transported to the hospital. Employee #1 was hospitalized with fractures.
