BROKEN ARM AND LACERATIONS-HAND CAUGHT IN CONVEYOR BELT

At 3:00 a.m., Employee #1 was training Employee #2 for duties related to his new maintenance position. During the training, Employee #1 observed a piece of broccoli stuck to a conveyor belt assembly, which was still running at the time. Employee #1 began to remove the broccoli with her gloved hand. As she began pulling the broccoli out, her hand became caught in the conveyor belt roller mechanism. Usually, a barrier guard is attached in front of the roller. However, the guard was broken, and a new guard was not installed at the time of the incident. She struggled to break free, but was unsuccessful. She shouted for help, and Employee #2 froze for a few seconds and then rushed to push the "stop" button. Employee #1 had a broken right arm and lacerations.
