Investigation Summary

Employee #1 was unable to back his beer delivery truck and trailer (approximately 49 ft long) down a driveway. The driveway was occupied by another delivery truck and there was a concrete wall on the opposite side. A driver of a third delivery truck attempted to back the Employee #1's truck down the driveway while Employee #1 stood on the outside. The third driver saw the victim in the passenger side view mirror standing at the back of the truck. As he started backing, the third driver looked in his drivers'-side mirror. The driver of the parked truck came out of the building and saw Employee #1 lying under his parked truck between the tractor and trailer. The second driver ran around the truck and found that Employee #1 was injured. The third driver at this time broke his side-view mirror on the concrete wall and stopped. The third driver saw the second driver and Employee #1 at the parked vehicle. The third driver got out of the truck and came over to the second driver and asked if he (the third driver) had run over Employee #1. Medical assistance was summoned and both Employee #1 and the third driver were taken to the hospital, Employee #1 for his injuries and the third driver for shock. Employee #1 was pronounced dead at the hospital of multiple traumatic injuries to his check, abdomen, ribs, lung and pelvis.
