Fall from extension ladder

On March 26, 1999, Employee #1 had climbed a ladder to the roof and a building where remodeling was occurring. Employee #1 asked a coworker to obtain a ladder and bring it to the rear of the building. A Werner extension ladder with a maximum working length of 17 feet was found and the coworker placed it against the roof edge of the building and climbed the ladder to the roof. The ladder was not tied off when placed against the building to gain access to the roof. The roof height is 12 feet, 2 inches. The coworker later descended the ladder and held it at the bottom placing his hands on the rails to support it for Employee #1's descent. While the coworker was looking forward when he heard a "click, click" sound familiar with the click that is made when the ladder is being adjusted upward. The coworker then looked up and saw the ladder retracting. Employee #1 was on the ladder at this time and swinging around at the top. Employee #1 later reported that he had grabbed the ladder to ensure it was stable before beginning his descent. The ladder then fell away from its resting place on the roof edge and struck the wall of the building. One of Employee #1's legs was then caught between two rungs, preventing it from retracting further. Employee #1 struck his head on the edge of the roof and received other injuries from impacting the ground and was hospitalized.
