Investigation Summary
At approximately 10:30 a.m. on May 21, 1999, Employee #1 was cutting custom-trim woodwork when he became distracted. He pushed the wood forward with his hand. His hand made contact with the saw blade. Employee #1 sustained multiple severe lacerations to the ring and middle fingers of his left hand and was taken to the hospital.
