EMPLOYEE INJURED IN FALL OFF LADDER

Employee #1 was setting a sheet of plastic on top of some carpeting inside a house under construction before detailing some windows at about 18 feet above the floor. He then placed an extension ladder on top of the plastic and leaned the top against the wall below the window. He instructed a laborer to hold the bottom of the ladder to secure it in place, while he climbed the ladder and performed some work above. Employee #1 had successfully completed three windows and was just finishing the fourth, when he noticed something outside and mentioned it to the laborer. The laborer let go of the ladder to look out the door to see what Employee #1 had told him. The plastic slipped on the carpet and the ladder shot out from under Employee #1 and he plunged to the floor, injuring his back.
