Employee suffers multiple fractures in fall off roof

At approximately 3:30 p.m. on April 16, 1997, Employee #1, a journeyman carpenter, was working at a construction site at Stanford University, where they were constructing a roof of 4 by 10 beams and 2 by 6 rafters. Employee #1 and three coworkers were laying down 5/8 or 3/4 in. plywood when the job manager realized that shear blocks had not been installed in the roof structure. The foreman told Employee #1's crew to rip out sections of the plywood and to install the missing blocks. As a result, there was a large amount of sawdust and some of the plywood sheets had nails protruding from them. Employee #1 was in the corner, snapping out lines on the plywood, when he tripped on a nail and slid on the sawdust. He fell approximately 20 ft off the edge of the roof, fracturing his hip, both sides of his pelvis, and both arms. Neither Employee #1 nor his coworkers were wearing a safety belt and lanyard, and none of this equipment had been supplied. There were no handrails or guardrails set up on the roof.
