Investigation Summary
On November 22, 2002, Employee #1, an electrician, was installing a light over the automatic teller machine (ATM). He was using an 8-ft ladder to access a junction box in the 11-foot ceiling. The light was connected to the emergency lighting system. A coworker noticed that Employee #1 was in the ceiling, up to his waist. The coworker later heard a sound and turned to see Employee #1 fall off the ladder, head first to the floor. He died at the site.
