Investigation Summary

Investigation Nr: 202313268
Event: 02/05/2002
Employee injured after tank fell onto employee from truck

At approximately 12:00 pm, Employee #1 of Reed Mariculture was trying to unload a 1030 pound compressed carbon dioxide gas cylinder alone from the back of a pickup truck. When the tank became unsecured, it began to move and got away from him. As it gathered momentum, the employee under-estimated the moving weight of the cylinder, and tried to grab and restrain it. The cylinder continued to roll, he thought he could grab the handle, and twist the wheels. The tank then rolled off of the truck and fell onto Employee #1. He was hospitalized for fractures.
