Investigation Summary
On or about 2:00 a.m. Employee #1 was unloading bulk food products from a truck to the receiving dock of the restaurant being serviced. He was using a mechanically attached "pull-out" step ladder (about 4 ft and 4 in. high). As he unloaded, he was standing on the top portion of the step ladder; as he began to descend, the platform suddenly collapsed, and he fell to concrete, sustaining a fractured right ankle. He was hospitalized for his injuries.
