Investigation Summary

At approximately 1:30 p.m. on August 10, 2001, Employee #1 was unstrapping several motor machinery components he had just delivered to Triangle Farms, as instructed by his supervisor. Another employee was operating a forklift. While Employee #1 was unstrapping the attached slings, the forklift driver accidentally hit the controls of the forklift to raise the blades. As the blades were rising, they struck Employee #1's hand, amputating his fingers. Employee #1 was hospitalized to treat his fingers.
