Investigation Summary

Investigation Nr: 202312807
Event: 12/06/2000
Employee's hand was amputated

Approximately 10:30 a.m., an employee was working as assigned to "feed loads" onto the company's Wadkin molder machine. The blades had been adjusted earlier by the Foreman to cut "wide loads"; however, at this point the employee was feeding in "narrow loads". A jam occurred at the entry point of the cutting head blade and the machine stopped. The employee, who had watched others correct such problems before moved to unjam the machine. Accordingly, he placed his left hand on the jammed piece of wood to pull it loose, bent over, and pushed the wood in to restart motor. His entire left hand was instantly pulled into the unguarded entry point of the blade and totally amputated up to the wrist. He was hospitalized for his injuries.
